New Student Paperwork

Welcome to PHAME! We are so excited for you to join us.

In order to be ready for PHAME classes, we’ll need you to fill out the following forms:

Student Information Form
Student Release Form
Tuition Assistance Request Form (optional)
Registration Form

We also ask that you review our Code of Conduct.

All students registering for in-person classes are asked to please review our Wellness & Covid Policies. Note that PHAME no longer requires proof of Covid vaccination, however we strongly encourage our community to be vaccinated for Covid and the flu.

Student Information Form

The Student Information form collects important information about your identity, how to contact you, accommodations that will help you participate at PHAME, and what to do in case of an emergency.

Deadline: As soon as possible. PHAME cannot process your registration until after we receive this form.

Student Release Form

The Student Release Form gives PHAME permission to use your likeness (pictures, videos, and other representations of you and your art) and also releases PHAME from liability.

Click here to download the form as a PDF. You must email or mail the signed copy back to PHAME—see instructions on the form.

Deadline: The Monday that falls one week before the term begins.

Tuition Assistance Request Form (Optional)

The Tuition Assistance Request Form collects information about your financial situation, and helps PHAME determine how much Tuition Assistance to award you. If you are able to pay full price for PHAME classes, you do not need to fill out this form.

Registration Form

If registration is currently open for the upcoming term—and if you haven’t already registered for classes with Alex—fill out our Registration Form. This form tells PHAME what classes you’d like to take.

Click here to visit our registration page. If registration is open for the upcoming term, you’ll find a button that links to the registration form.

Deadline: New students get first dibs on classes. If registration is open, we’ll process your registration form as soon as we receive it. So the sooner you submit it, the better.

We’ll send you an email after we process your registration, and that email will list what classes you’ve been placed in and how much you owe. If we’re awarding you Tuition Assistance, we’ll send you a separate email confirming that amount.
